Step #1: Read & Sign Our Client Agreement

To become a client of Jen McCulloch, you will need to read and agree to our Client Agreement. This agreement includes an Informed Consent Form as well. Before starting any training program with us, you are required to read and sign the Informed Consent Form. This form explains the details of the program and any potential benefits or risks that may be associated with it.

Step #2: Complete Your New Client Intake Form

The New Client Intake Form collects some basic information about you and your needs. This will enable us to provide you with the best possible service. Please take a few minutes to complete the form. This will ensure that Jen has all the information necessary to provide you with high-quality service. Thank you for taking the time .

Step #3: Set Up Your Payment Details

To proceed with your agreed service with Jen McCulloch, you need to authenticate your payment details using Stripe. You will be set up a weekly payment every Thursday by following the attached link, entering your card details, and paying a $1 setup fee. This will authorise Jen McCulloch to initiate regular payments.
