Step #1: Read & Sign Our Client Agreement
To become a client of Jen McCulloch, you will need to read and agree to our Client Agreement. This agreement includes an Informed Consent Form as well. Before starting any training program with us, you are required to read and sign the Informed Consent Form. This form explains the details of the program and any potential benefits or risks that may be associated with it.
Step #2: Complete Your New Client Intake & Assessment Form
Client Intake & Assessment Form helps us get to know you, your goals, and your lifestyle.
This ensures Jen can design a coaching plan that fits you, your body, your schedule, and your mindset.
Please take some time to complete this form. It includes your training history, health details, access to equipment, and how you’d like to be supported if things go off track.

Step #3: Set Up Your Payment Details
To proceed with your agreed service with Jen McCulloch, you need to authenticate your payment details using Stripe. You will be set up a weekly payment every Thursday by following the attached link, entering your card details, and paying a $1 setup fee. This will authorise Jen McCulloch to initiate regular payments.
